Imagine you are unable to speak or make choices for your own life, will you allow your loved ones to make tough healthcare decisions for you?

The national Advance Care Planning (ACP) programme was launched in 2011 to encourage Singaporeans to discuss and share their care preferences with loved ones in advance through an ongoing process of communication. It is now carried out at general public hospitals island-wide.

In this awareness talk about Advance Care Planning, participants will:

1. Better understand the life choices one may face when critically ill
2. Listen to real-life stories of struggles experienced by patients before death
3. Understand how to do up a future healthcare plan
4. Explore differences between Advance Care Planning, Advanced Medical Directive and Lasting Power of Attorney
