Then, there are also the decisions that are always too early to talk about until it is too late — if you would to suffer a heart attack that leaves you unconscious or unable to recognise your loved ones, would you want to permanently remain on life support?

Advance Care Planning is a national programme targeted at all Singaporeans, the young and old, the healthy and ill, to make these decisions early and share them with loved ones. This plan is a gift of love to them so that they will know how best to care for you and honour your preferences.
