Imagine you were unable to speak or make choices for your own life, would you allow your loved ones be left to make decisions for you?

Some decisions may be easy — who will collect parcels and water your plants on your behalf? Others are less so — who will bathe you if you are unable to do it yourself? Then, there are also the decisions that are always too early to talk about until it is too late — if you would to suffer a heart attack that leaves you unconscious or unable to recognise your loved ones, would you want to permanently remain on life support?

Advance Care Planning is a national programme targeted at all Singaporeans, the young and old, the healthy and ill, to make these decisions early and share them with loved ones. This plan is a gift of love to them so that they will know how best to care for you and honour your preferences.
